Question about withdrawing bonus from Desert Nights Casino

Hi, I didn't quite know where to put this.. but here goes:

 

Today i managed to play through a bonus at Desert Nights Casino, i got the status "Cashout requirements are met"

I have to make a deposit to claim the money i won, as well as supply documents to verify me.

 

That is all ok, but one of the documents i was asked to provide is a form i have to fill out..

I am not quite sure what it implies.

 

I have attached the form, which is a pdf file.

 

What exactly am i signing when i fill out ths form and sign it to get my withdrawal?

is it safe ?

 

 

i would greatly appreciate it if someone could help me look this over. :-)

I just want to ask you first if you read carefully their terms and condition about the bonus? Also about the verification of your account to them? That was a modern form of verifying an account i don't know why do they need this one cause i only seen this form if you're from a strict country that needs security verification like credit cards in order deposit to online casinos.

Fill out the form scan it and send it with the rest of the documents. I'ts a standard step in first time withdrawals and you do it only once in one casino. If you played according to the bonus terms, like allowed games and bet size limit your withdrawal should go without problems.

Today i recieved €170 in my bank account. I won the money just by playing a €10 no deposit bonus on the slot machine "Mystic Wolf".

 

After the form was e-mailed to their customer support. The withdrawal was processed within a few days. 

I  got the withdrawal accepted without having to deposit anything.

 

I was actually suprised to see everything go so well.

 

Just wanted to let you guys know how it turned out!
